MYWAYV WELLNESS PILOT STUDY

PARTICIPANT CONSENT AND PRIVACY NOTICE

Please read this document carefully before participating.

1. ABOUT THE STUDY

This wellness pilot study is conducted by MyWayv Holding Inc., a Delaware corporation, together with its affiliated entities, including its Israeli subsidiary ("MyWayv," "we," "our," or "us").

The purpose of this study is to better understand how users experience the MyWayv wellness platform and whether users report changes in areas such as wellbeing, resilience, sleep quality, emotional balance, stress management, and daily functioning over time.

2. IMPORTANT NOTICE

The MyWayv platform is a wellness product only.

It is not a medical device, healthcare service, therapy platform, or mental health treatment program.

Participation in this study does not constitute medical care, psychological treatment, or professional advice.

The MyWayv platform is not intended to diagnose, treat, cure, prevent, or monitor any disease, medical condition, or mental health condition.

If you are experiencing a medical emergency, mental health crisis, or believe you may be a danger to yourself or others, seek immediate assistance from a qualified healthcare professional or emergency services.

3. WHAT PARTICIPATION INVOLVES

If you choose to participate, you will be asked to:

• Use the MyWayv app during a 14-day study period.

• Complete three short questionnaires:

• Day 1

• Day 7

• Day 14

Each questionnaire takes approximately 5 to 10 minutes to complete.

Participation is entirely voluntary.

4. VOLUNTARY PARTICIPATION

Your participation is completely voluntary.

You may stop participating at any time and for any reason.

You may skip any survey question you do not wish to answer.

Choosing not to participate or discontinuing participation will not result in any penalty or loss of benefits.

5. ELIGIBILITY

By participating, you confirm that:

• You are at least 18 years of age.

• You are participating voluntarily.

• You have read and understood this document.

6. DATA WE COLLECT

For purposes of this study, we may collect:

• Questionnaire responses.

• Limited technical information necessary to administer the study.

We do not intentionally collect medical records, financial information, biometric information, genetic information, or other sensitive personal information as part of this study.

Voice conversations, stories shared within the app, and user interactions with the conversational experience are not collected or analyzed as part of the study results.

7. HOW YOUR INFORMATION IS USED

Information collected during this study may be used to:

• Evaluate and improve the MyWayv platform.

• Understand participant experiences and wellness trends.

• Prepare aggregated statistical reports.

• Support business, research, educational, investor, marketing, and product development activities.

Any reporting or publication of study results will use aggregated data and will not intentionally identify individual participants.

We do not sell participant information.

8. PRIVACY AND DATA STORAGE

MyWayv uses reasonable administrative, technical, and organizational safeguards to protect study information.

Study information may be stored and processed in the United States, Israel, or other locations where MyWayv or its service providers operate.

Information collected from participants in Israel may be transferred to and processed in the United States.

9. DATA RETENTION

Study information may be retained for research, compliance, operational, and business purposes for a reasonable period consistent with applicable law.

Where data has been anonymized or aggregated, it may continue to be used for statistical and research purposes.

10. PARTICIPANT RIGHTS

Subject to applicable law, participants may request information regarding personal data held by MyWayv and may request correction, deletion, or withdrawal of consent where applicable.

11. NO GUARANTEE OF RESULTS

Individual experiences vary.

MyWayv makes no promise, guarantee, or representation that participation in this study or use of the platform will produce any specific result or improvement.

12. RELEASE AND LIMITATION OF LIABILITY

By participating, you acknowledge that:

• Participation is voluntary.

• The MyWayv platform is a wellness tool and not a medical or therapeutic service.

• You are responsible for your own decisions regarding health and wellbeing.

To the fullest extent permitted by law, you release and hold harmless MyWayv Holding Inc., its affiliates, officers, directors, employees, contractors, agents, and representatives from claims arising from your voluntary participation in this study, except where liability cannot legally be excluded.

2. חשוב לדעת

אפליקציית

MyWayv

היא אפליקציית רווחה אישית בלבד.

האפליקציה אינה מכשיר רפואי, טיפול נפשי, שירות רפואי או תחליף לייעוץ מקצועי.

המחקר אינו מחקר רפואי ואינו ניסוי קליני.

האפליקציה אינה מיועדת לאבחן, לטפל, לרפא, למנוע או לנטר מחלה או מצב רפואי כלשהו.

אם אתם נמצאים במצוקה נפשית משמעותית, במשבר, או חוששים לפגיעה בעצמכם או באחרים, יש לפנות מיד לגורם מקצועי מוסמך או לשירותי החירום המתאימים.

3. מה כוללת ההשתתפות

אם תבחרו להשתתף במחקר, תתבקשו:

• להשתמש באפליקציה במשך כ-30 ימים.

• למלא שלושה שאלונים קצרים:

• בתחילת המחקר

• באמצע המחקר

• בסיום המחקר

משך מילוי כל שאלון הוא כ-5 עד 10 דקות.

4. השתתפות מרצון

ההשתתפות במחקר היא מרצונכם החופשי בלבד.

ניתן להפסיק את ההשתתפות בכל עת וללא צורך במתן הסבר.

ניתן לבחור שלא להשיב על שאלות מסוימות.

ההשתתפות במחקר אינה קשורה למעמדכם בארגון, במקום העבודה או במסגרת כל תוכנית שבה אתם משתתפים, ולא תשפיע על זכאותכם לשירותים או להטבות כלשהן.

5. תנאי השתתפות

בהשתתפותכם אתם מאשרים כי:

• גילכם 18 שנים ומעלה.

• אתם משתתפים מרצונכם החופשי.

• קראתם והבנתם את תוכן מסמך זה.

6. מידע שנאסף במסגרת המחקר

במסגרת המחקר עשויות להיאסף תשובותיכם לשאלונים ומידע טכני בסיסי הנדרש לצורך ניהול המחקר.

שיחות קוליות, שיחות עם האפליקציה ותכנים שתבחרו לשתף במהלך השימוש באפליקציה אינם חלק מניתוח תוצאות המחקר.

7. שימוש במידע

המידע שייאסף עשוי לשמש לצורך:

• שיפור האפליקציה וחוויית המשתמש.

• הערכת חוויית המשתמש והשפעת השימוש במערכת.

• הפקת נתונים סטטיסטיים מצרפיים.

• מחקר ופיתוח, מצגות, פרסומים מקצועיים, חומרים למשקיעים וחומרי הסברה ושיווק.

לא יפורסם מידע המזהה משתתף באופן אישי.

החברה אינה מוכרת מידע אישי של משתתפים לצדדים שלישיים.

8. פרטיות ושמירת מידע

החברה נוקטת באמצעים סבירים להגנה על המידע הנאסף במסגרת המחקר.

המידע עשוי להיות מאוחסן או מעובד בישראל, בארצות הברית או באמצעות ספקי שירות הפועלים מטעם החברה.

מידע הקשור למחקר עשוי להיות מועבר ולעבור עיבוד בארצות הברית.

9. זכויות המשתתפים

בכפוף להוראות הדין החל, ניתן לפנות לחברה בבקשה לקבל מידע לגבי נתונים אישיים שנשמרו, לבקש תיקון מידע או לבטל הסכמה, ככל שהדבר אפשרי.

10. היעדר התחייבות לתוצאה

תוצאות השימוש באפליקציה עשויות להשתנות מאדם לאדם.

החברה אינה מתחייבת להשגת תוצאה מסוימת או לשיפור כלשהו בעקבות ההשתתפות במחקר או השימוש באפליקציה.

11. הגבלת אחריות

בהשתתפותכם במחקר אתם מאשרים כי:

• ההשתתפות היא מרצונכם החופשי.

• ידוע לכם כי מדובר באפליקציית רווחה ולא בשירות רפואי או טיפולי.

• האחריות להחלטות הנוגעות לבריאותכם ולרווחתכם האישית נותרת בידיכם.

בכפוף להוראות כל דין, לא תהיה לחברה, לנושאי המשרה שלה, לעובדיה, לקבלניה או לנציגיה אחריות לטענה או לנזק הנובעים מהשתתפותכם מרצון במחקר זה.
